Pay Grade

B9 ($27.26 – $39.76 hourly, depending on experience)

Potential for 10% bilingual pay increase—must pass a language test after hire. $5,500 Hiring Bonus.

Overview

Provides case management services to individuals in the Developmental Disabilities program in Morrow County, following Oregon Administrative Rules. Coordinates additional services and activities within the program and performs tasks as directed by the DD Site Supervisor, DD Program Manager, or Chief Operating Officer. Requires travel within the County; an agency vehicle is provided.

Responsibilities
  • Assess client eligibility per Oregon Administrative Rules.
  • Provide case management services across diverse community settings.
  • Maintain, submit, and ensure the timeliness of all required documentation.
  • Assist clients in securing services, resources, and support for independence.
  • Coordinate with community partners to offer comprehensive services.
  • Provide transportation when necessary.
  • Attend local, regional, and state meetings as requested.
  • Act independently and promote a positive agency image; attend team and agency meetings.
  • Stay updated on Individual Service Plan (ISP) meetings and objectives.
  • Meet monthly billable services contact requirements, unless otherwise arranged.
  • Investigate abuse reports and implement protective services.
  • Act as broker to ensure clients meet required contacts (e.g., psychiatrist, medical doctor, Supplemental Social Security Insurance).
  • Participate in joint case planning and coordination with other CCS staff.
  • Maintain knowledge of all relevant Rules and Oregon Administrative Rules for DD Services.
  • Ensure contractors/providers monitored by CCS comply with applicable OARs.
  • Provide consultation to on‑call staff supporting individuals with developmental disabilities.
  • Input necessary data into CCS databases for DD program clients.
  • Assist DD Program Manager with recruitment and maintenance of Lake County Advisory board members.
  • Other duties as assigned.
Requirements
  • Meet the minimum standards for Service Coordinator/Case Manager under Community Developmental Disability Programs as defined in OAR 411‑320‑0030 and 411‑415‑0040.
  • Bachelor’s degree in behavioral science, social science or closely related field; OR bachelor’s degree in any field with one year of human services experience; OR associate’s degree in behavioral science/social science with two years of human services experience; OR three years of human services experience.
  • Possess a valid driver’s license and personal auto insurance with required liability coverage.
  • Learn and implement DD case management standards; understand DD services, resources, and funding sources.
  • Strong verbal and written communication skills.
  • Work independently and complete responsibilities thoroughly and timely.
  • Computer literate—proficiency with word processing and other software.
  • Maintain confidentiality of client information in accordance with laws and regulations.
  • Pass criminal background checks and pre‑employment drug screen.
  • Physical demands: may lift up to thirty pounds; requires walking, standing, reaching, use of office technology; ability to sustain attention during crisis situations.
  • Work environment: office and community; frequent travel within and outside the county; overnight stays for meetings and trainings.
