Position Details
JOB TITLE:
Developmental Disability Services Coordinator
LOCATION:
Lakeview, OR – Lake County
FLSA: 1.0 FTE (40 hours/week, Monday‑Friday)
SUPERVISOR:
Regional Supervisor / DD Program Manager
PAY GRADE: B9 ($27.26 – $39.76 per hour, depending on experience)
$5,500 Hiring Bonus (taxed, paid in two staggered payments)

- Assess eligibility of potential clients using guidelines from the Oregon Administrative Rules.
- Provide case management services in various community settings, respecting client preferences.
- Maintain, submit, and ensure timeliness of all records, case notes, reports, and other documentation.
- Help clients acquire services and resources to achieve goals and maintain community independence.
- Coordinate with community partners to develop, provide, and uphold a comprehensive array of services.
- Provide transportation for clients when required.
- Attend local, regional, and state meetings as requested by the supervisor.
- Act independently within the community and promote a positive image of the agency.
- Participate in all team and agency meetings as scheduled.
- Keep updated on all Individual Service Plan (ISP) meetings and objectives.
- Meet all billable service contact requirements monthly, unless otherwise arranged through the Executive Director.
- Investigate abuse reports and implement protective services for all clients when necessary.
- Act as a broker to ensure clients meet required contacts (e.g., psychiatric consults, medical visits, Supplemental Social Security benefits).
- Participate in joint case planning and coordination with other CCS employees.
- Stay informed on all rules and OARs related to Developmental Disabilities services.
- Ensure monitored contractors and providers comply with relevant OARs.
- Provide consultation to on‑call staff supporting individuals with developmental disabilities.
- Enter all necessary information into CCS databases for clients in the DD program.
- Assist the DD Program Manager with recruitment and maintenance of Lake County Advisory Board members.
- Other duties as assigned.
Education and Experience
- Bachelor’s degree in behavioral science, social science, or a closely related field; or
- Bachelor’s degree in any field plus one year of human‑services experience; or
- Associate’s degree in a related field plus two years of human‑services experience; or
- Three years of human‑services experience; or
- Variance may be requested for candidates not meeting the above requirements.

- Ability to learn and apply DD case‑management standards and resources.
- Strong verbal and written communication skills.
- Independent worker who completes responsibilities meticulously and on time.
- Representative of client and agency interests in a positive manner.
- Valid driver’s license and suitable insurance for client service travel; must pass criminal background checks.
- Computer literacy, including word processing and relevant software systems.
- Ability to handle highly confidential information in accordance with client‑confidentiality laws.
- Must pass all criminal history checks required by ORS 181.536‑181.537 and OARs 410‑007‑0200 to 410‑007‑0380.
- Monthly checks against OIG, GSA exclusion lists, and other federal/state lists.
- Pre‑hire drug screening is required.
- Must hold a valid driver’s license and personal auto insurance for CCS business use (≥$300,000 liability coverage).
- Regular use of computer and phone requiring fine motor skills.
- Tasks include sitting, talking, listening, typing, reaching, standing, walking, stooping, kneeling, and occasional lifting up to 30 pounds.
- Requires vision and attention during crisis situations.
- Work occurs at office and in community settings, with frequent travel within Lake County and possible out‑of‑area/overnight travel for meetings and training.
- May work in uneven terrain and respond to crisis environments lacking handicap access.
