Position Summary

We are seeking a compassionate and organized Social Services Director to oversee and provide medically related social services that support the emotional, social, and psychosocial well‑being of our residents. This role collaborates closely with residents, families, and the interdisciplinary team to ensure quality care and successful outcomes.

Key Responsibilities
  • Assess residents’ social, emotional, and family needs and develop appropriate care plans
  • Provide emotional support, counseling, and guidance to residents and families
  • Assist residents in achieving the highest level of independence and well‑being
  • Coordinate with community agencies and resources to meet resident needs
  • Support discharge planning, including referrals, equipment, and services
  • Schedule and coordinate outside services such as podiatry, dental, psychiatric care
  • Assist residents and families with care decisions and transitions
  • Maintain accurate documentation of social services interventions
  • Respond to concerns regarding resident belongings and personal needs
  • Collaborate with interdisciplinary team members to ensure quality care
  • Provide staff training and in‑services as needed
Qualifications
  • Bachelor’s degree in Social Work or Human Services (required)
  • MSW preferred
  • Minimum 2 years of supervised social work experience in a healthcare setting
  • Experience in skilled nursing or long‑term care preferred
  • Strong communication, organization, and documentation skills
  • Ability to handle sensitive information with confidentiality and professionalism
