Human Services Case Manager
Position:
Human Services Case Manager – Full‑time, permanent, with the Oregon Department of Human Services. Determine initial and ongoing Medicaid eligibility for long‑term services and supports (LTSS). Develop and implement service plans, coordinate community‑based care and nursing home services, and review and update eligibility and plans regularly.
Minimum Qualifications- Bachelor’s degree in Behavioral Science, Social Science, or a closely related field.
- OR bachelor’s degree in any field plus one year of relevant human services experience (e.g., assisting economically disadvantaged, employment, abuse and neglect, substance abuse, aging, disabilities, prevention, health, cultural competencies, inadequate housing).
- OR associate degree in a related field plus two years of related human services experience.
- OR three years of related human services experience.
- Excellent communication skills to discuss/assess client needs.
- Strong people/customer service skills.
- Proficient in obtaining information through interviews.
- Ability to prepare and maintain written documentation of actions taken.
- Office hours 8a.m. – 5p.m., Monday–Friday, but may require travel to homes, hospitals, nursing homes, and local offices.
- May involve physical exertion: climbing stairs, walking uneven terrain, transporting a laptop.
- Exposure to environmental hazards, unsanitary conditions, potential hostility, animal contact, and contagious diseases (e.g., MRSA, COVID‑19). Must have a valid driver’s license and acceptable driving record.
Selected candidates will undergo a criminal history and background check. Adverse findings may lead to disqualification. The position does not sponsor visas.
