Summary of Duties

As a Human Services Case Manager, you will:

This position will match the needs of Aging and People with Disabilities in the community with current federal, state and local programs or other resources that are available to meet those needs. This is done through case management and by screening applicants for long term community-based care and nursing homes. This position will also assess eligibility, re-determine needs, and adjusts benefits and services accordingly.

Also, this individual will act as an advocate for clients and aging and persons with disabilities within the community.

Minimum Qualifications

A valid driver's license and acceptable driving record are required for this position.

  • A bachelor's degree in a Behavioral Science, Social Science, or a closely related field.

  • OR a bachelor's degree in any field AND ONE YEAR of human services related experience (i.e., work providing assistance to individuals and groups with issues such as economically disadvantaged, employment, abuse and neglect, substance abuse, aging, disabilities, prevention, health, cultural competencies, inadequate housing).

  • OR an associate degree in a Behavioral Science, Social Science or a closely related field AND TWO YEARS of human services related experience (i.e., work providing assistance to individuals and groups with issues such as economically disadvantaged, employment, abuse and neglect, substance abuse, aging, disabilities, prevention, health, cultural competencies, inadequate housing).

  • OR Three years of human services related experience (i.e., work providing assistance to individuals and groups with issues such as economically disadvantaged, employment, abuse and neglect, substance abuse, aging, disabilities, prevention, health, cultural competencies, inadequate housing)

Essential Attributes

  • Experience screening and assessing candidates through interviewing to determine initial eligibility for Long Term Care Services (LTCS)

  • Experience developing detailed individualized service plans

  • Experience reaching out to community referrals for other services including Adult Protective Services

  • Experience documenting all aspect of the cases in narration

Working Conditions

  • This position requires work on site and is not a remote position.

  • The office is open from 8am-5pm, Monday through Friday. The needs of the public will be adequately served.

  • The work schedule for this position is a 40-hour work week. May require some overtime work before and after scheduled work hours and on weekends.

  • General office conditions. This position also includes an office environment that includes long periods of sitting for data entry, document reviews and desk side interviewing.

  • Daily ongoing use of a keyboard and CRT screen are necessary. There may be physical exertion such as climbing stairs, walking on uneven terrain and transporting a laptop.
