Social Worker - Addus Family Company Addus Home Care seeks a compassionate Social Worker to support seniors and individuals with disabilities in home and community settings. In this role, you will complete psychosocial assessments, develop person-centered care plans, coordinate services, and advocate for client needs so they can remain safe and independent will collaborate closely with nurses, caregivers, and families, providing counseling, education, and resource referrals.
