Position Highlights- Full-time, outpatient primary care
- Guaranteed salary with earning potential of $300k+
- 4-day / 10-hour schedule
- No nights or weekends
- Combination of in-office visits, telehealth, and limited same-day appointments
- Functional, preventive, and integrative care model

- Collaborative care teams including health coaches, nutritionists, and mental health providers

- Provide comprehensive primary care through office visits and telehealth
- Perform assessments, order and interpret labs and imaging, and manage acute and chronic conditions
- Develop personalized treatment plans incorporating lifestyle interventions, supplements, hormone therapy, and prescriptions as appropriate
- Educate and empower patients using preventive and functional medicine principles
- Collaborate closely with multidisciplinary care teams
- Maintain accurate and timely documentation in the EHR
- Licensed Nurse Practitioner (FNP, AGNP, AGNP-PC) or Physician Assistant (PA-C)
- Oregon license or ability to obtain
- Board certification in Family Medicine, Internal Medicine, or relevant specialty
- Minimum 2 years of clinical experience preferred
- Strong interest in preventive, functional, and lifestyle medicine
- Excellent communication skills and comfort in a collaborative environment
