Within this high-performing Case Management and Social Services service line at a hospital designated as a Stroke Center and Cardiac Center with a strong Orthopedics program, the Manager will guide utilization management, discharge planning, complex transitions of care, and psychosocial services that reduce length of stay and readmissions while advancing patient-centered outcomes.
Key Responsibilities- Drive staffing and workflows, mentor leaders, and ensure compliance with CMS Conditions of Participation and Joint Commission standards.
- Collaborate on denials management and payer relations, and champion evidence-based, interdisciplinary care pathways.
- Partner closely with bedside nurses, Social Workers, RN Case Managers, Hospitalists, Surgeons, Pharmacy, Therapy, and Revenue Integrity to support clinical teams.
- Report to the Director of Care Coordination within Nursing leadership and lead the Case Management and Social Services team.
- Graduated from an accredited school of professional nursing with a Bachelor of Science in Nursing (BSN) or a master’s degree in social work (MSW).
- Current and valid RN License in the state of Oregon OR Preferred LCSW/LMSW in the state of Oregon (if an MSW is held).
- Case Management CCM or CPHQ Certification preferred with at least 2 years of experience in case management and supervisory within a hospital or healthcare setting.
