Care Coordinator – Licensed

• Perform comprehensive member assessments, including face-to-face and in-home visits
• Develop and implement individualized care plans with members, caregivers, physicians, and support networks
• Monitor care plans, document interventions, and adjust plans as needed
• Promote integration of behavioral health, LTSS, and community resources
• Evaluate benefits and advise on funding sources
• Facilitate interdisciplinary care team meetings and collaborate with team members
• Use motivational interviewing to educate and support members
• Identify barriers to care and address psychosocial, financial, and medical concerns
• Develop prevention plans for critical incidents to protect member health and safety
• Maintain accurate documentation in electronic systems and comply with standards
• Travel locally 25–40% for member visits
• Perform other duties as assigned

• Active, unrestricted Oregon Registered Nurse (BSN or MSN) license
• Qualifications meeting eligibility requirements to sit for the Certified Case Manager (CCM) examination
• Minimum of two years of healthcare experience
• At least one year supporting individuals with disabilities or chronic conditions in Long-Term Services and Supports (LTSS)
• At least one year of experience in care management or a medical and/or behavioral health setting
• Valid driver’s license, reliable transportation, and current automobile insurance
• Proficiency in Microsoft Office Suite
• Ability to navigate electronic health records and care management systems
• Strong knowledge of Oregon community resources
• Experience working with diverse populations
• Strong time management, multitasking, and problem-solving skills
• CCM certification required within 18 months of hire
• Knowledge of Medicaid and Medicare programs, including waiver services
• Familiarity with healthcare regulatory and compliance standards
• Experience providing culturally competent care to diverse and underserved populations
• Accurate electronic documentation experience
• Bilingual or additional language skills are a plus

Demonstrates expertise in care management, including developing and implementing individualized care plans, conducting comprehensive member assessments, and navigating electronic health records. Proficient in collaborating with interdisciplinary teams and utilizing community resources to support diverse populations.
