About the Role

Optum Home & Community Care is seeking a Nurse Practitioner or Physician Assistant to join our Senior Community Care (SCC) team. This role provides comprehensive, patient-centered care to seniors in assisted living, nursing homes, and community settings
, with a focus on quality outcomes—not volume.

You’ll work as part of an interdisciplinary team managing high-risk patients, blending internal medicine and palliative care
, with a manageable caseload of approximately 4–6 visits per day
.

Key Responsibilities
  • Manage acute issues and chronic condition exacerbations
  • Order and interpret diagnostics and treatments
  • Collaborate with interdisciplinary care teams
  • Lead advanced illness and care planning discussions
  • Educate patients and caregivers
  • Coordinate care transitions across settings
Qualifications
  • MSN or DNP from an accredited program
  • Active NP license (or ability to obtain)
  • Board certified (AANP or ANCC – FNP, Adult, Gerontology, or Adult-Gerontology)
  • Graduate of an accredited PA program
  • Active PA license (or ability to obtain)
  • NCCPA certification
  • DEA or ability to obtain (state dependent)
Additional Requirements:
  • Reliable transportation
  • Ability to meet physical job requirements
Preferred:
  • New graduates encouraged to apply
  • Experience in geriatrics, senior living, or home-based care
