Location: Jackson County, OR – must have OR license and live local to Jackson County

Role Overview

Ideal candidates thrive in a collaborative, compassionate community of caregivers. As a Nurse Practitioner at GGH, you will manage patients in an outpatient clinic setting and provide in‑home clinical support for community patients. You will partner with clinics as an embedded member of the care team, aiming to prevent unnecessary acute‑care admissions, provide holistic patient care, and address clinical and psychosocial factors that contribute to cyclical health‑care utilization.

What

You Will Do
  • Assess patients' clinical problems and healthcare needs and provide clinical care and consultation
  • Perform annual wellness exams for senior Medicare patients in clinic or at home, including medical history, symptom review, and medication reconciliation
  • Handle HCC coding, verification, and risk adjustment
  • Coordinate continuity of care, prevention, and avoidance of complications under the direction of the patient’s primary healthcare provider(s) and other referral sources
  • Perform various tests and screenings
  • Counsel patients on their discharge plans
  • Execute discharge plans and develop effective care plans during critical transitions after hospital discharge
  • Support patients and families with compassionate care
  • Contribute to an interdisciplinary team (IDT) model and attend regular IDT meetings with social workers, case managers, and other clinicians
  • Collaborate with the care team to ensure completion of holistic patient care
  • Travel within the Medford, OR area (up to 35 miles) five days a week for home visits as needed
  • Complete charts within 72 hours of patient visits; use EMR portals for chart review and documentation
  • Cover for other Nurse Practitioners and team members when necessary
  • Maintain patient confidentiality and comply with all HIPAA rules

This full‑time position (Monday–Friday, 8 a.m.–5 p.m.) may include in‑clinic, in‑home, or telehealth visits. Home visits will be scheduled within a 30‑mile radius of your home region (Jackson County, OR).

  • Flexible with work schedule and setting as needed
  • Travel within a 35‑mile radius of Jackson County, OR five days a week as business needs dictate
Experience & Qualifications
  • Minimum 2 years of clinical experience as an NP in primary care, family medicine, internal medicine, gerontology, or palliative care
  • Experience with acute care and high‑risk patients is a plus
  • Home health experience (NP or RN) is a plus
  • Nurse Practitioner certification from ANCC or AANP
  • Working knowledge of chronic care management and medication options
  • DEA licensure / prescriptive authority post‑hire
  • Active CPR/BLS certification
