• Competitive hourly pay: $45–$55/hour (DOE)
• Mileage reimbursement for client visits

• Perform in‑home client assessments and re‑assessments
• Provide nursing services within RN scope of practice
• Support development and updates of client care plans
• Monitor client status and communicate changes to the clinical team
• Document all visits accurately and in a timely manner
• Collaborate with the Clinical Manager and care team to ensure quality care
• Provide patient education and support to clients and families

• Must be able to legally work in Oregon, USA to apply for this position
• Previous experience in home health, hospice, or acute care preferred
• Strong clinical judgment and assessment skills
• Strong understanding of In‑home care and home health regulations
• CPR certification and TB clearance required
• Valid driver’s license, insurance, and reliable transportation
• Strong communication, organization, and clinical judgment skills
