Responsibilities and Duties:
  • Provide clinical oversight and supervision of caregivers and nursing staff
  • Conduct initial and ongoing in‑home client assessments
  • Develop, implement, and update individualized care plans
  • Provide nursing services within scope of practice
  • Ensure compliance with Oregon HH and IHC regulations and agency policies
  • Support care coordination with physicians, families, and external care partners
  • Assist with staff onboarding, training, and clinical mentoring
  • Participate in quality assurance and performance improvement activities
Requirements:
  • 2+ years nursing experience (home health, hospice, or related field preferred)
  • Leadership or supervisory experience required
  • Strong understanding of In‑home care and home health regulations
  • CPR certification and TB clearance required
  • Valid driver’s license, insurance, and reliable transportation
  • Strong communication, organization, and clinical judgment skills
  • Current TB Screen and Criminal Record Check
  • Must be able to legally work in Oregon, USA to apply for this position
