Home Health Clinical Triage - Registered Nurse

About This Role

As a Home Health Clinical Triage Specialist, you'll play a vital role in delivering excellent patient care through triage calls after hours and on holidays. This position is ideal for nurses who are dedicated to providing compassionate care and can work independently.

• Pay: $33.00 to $49.50 per hour
• Location: Lafayette, LA
• Schedule: Per Diem, must be available to work weekends

• Receive calls from patients and/or family members and assess problems.
• Assess patient status and intervene as indicated by the patient s condition.
• Document calls received within the electronic medical records system.
• Utilize basic triage protocols to address patient needs in crisis situations.
• Coordinate with agency on-call nurses to schedule nursing assessments.
• Attend mandatory educational offerings and in-services for updates.
• Facilitate orientation of new personnel as assigned.
• Communicate effectively when assessing or educating patients/caregivers.
• Advocate for patients and demonstrate integrity in nursing practice.

Education:
Current, unrestrictive Registered Nurse licensed in all states of practice

• 2+ years of RN experience preferred
• 1+ years of direct patient care in a hospice, home health, or oncology setting preferred

• Current RN License required
• Registered Nurse Compact licensure preferred
