In-Home Nurse Practitioner or Physician Assistant Part Time - Eugene

Signify Health, part of CVS Health®, is seeking a Part‑time Clinician (Nurse Practitioner or Physician Assistant) to provide In‑Home Health Evaluations, engaging with people in the comfort of their own homes and helping bridge gaps in care.

The role involves building trusted relationships to improve health outcomes, delivering flexible, comprehensive and personalized evaluations at the member’s front door.

• Bring your heart into every visit, joining a national network of purpose‑driven clinicians dedicated to improving health outcomes.
• Participate in visiting millions of people nationwide to connect them to appropriate care.
• Work with Signify Health as a clinician colleague, fulfilling the specified role requirements.
• Be flexible to travel locally and within licensed states, as needed.
• Conduct virtual visits as required.
• Provide all Signify Health services indicated, including Diagnostic & Preventive Services such as spirometry, diabetic eye exams, bone density screening, and more.
• Deliver a 45‑minute visit that includes a medication and medical history review, a physical evaluation, and optional diagnostic tests when ordered by the health plan.
• Use an iPad and an intuitive clinical workflow for each evaluation; prescribing medication, ordering lab tests, or altering current treatment regimens is not required.

• Active, unrestricted license(s) in the coverage area(s).
• Board certification (required).
• Multi‑state licenses (preferred).

Pay range: $41.85 - $90.13 per hour, based on experience, education, geography, and other factors. Anticipated weekly hours: 29.5. Time type:
Part‑time.
