: 8 a.m. to 5:30 p.m., Monday–Thursday; 8 a.m. to 12:00 p.m. on Friday. Additional hours may be required.
The Health Coach is part of the care team under the supervision of the Director of Health Education. The Health Coach works with the Diabetes Self‑Management team to assist the care team with direction and resources regarding diabetes self‑management aimed at meeting patient needs. The Health Coach integrates clinical and self‑management aspects of diabetes and cardiometabolic care to optimize quality care, promote self‑management, and achieve individualized behavioral and treatment goals.
The Health Coach assists in meeting organizational goals and participates in continuous Quality Improvement practices aligned with the Patient Centered Medical Home model.
- Coordinate communication between patients, families, and caregivers with the multidisciplinary care team to identify barriers and support the care plan.
- Integrate skills and knowledge of pathophysiology, epidemiology, clinical management, cardiometabolic conditions, and diabetes self‑management into clinical practice.
- Partner with individuals to deliver care and education conducive to behavior change and improved quality of life for pre‑diabetes, diabetes and cardiometabolic conditions throughout the lifespan.
- Provide quality diabetes self‑management education in individual and group settings based on assessed needs, using appropriate teaching techniques that respect learning preferences.
- Follow the ADCES Diabetes Education Accreditation Program requirements, which are based on the National Standard of Diabetes Self‑Management and Support Guidelines.
- Complete comprehensive assessments for each patient, including emotional and behavioral health, interpret personal health data, develop an individualized care plan based on assessed needs and goals, and promote successful self‑management.
- Document all individual contacts/visits in the EHR according to MHSI and Accreditation Program guidelines in a timely manner.
- Ensure patient confidentiality.
- Process data and compile reports on various projects.
- Notify and recall patients regarding missed appointments and care gaps.
- Communicate with patients and staff daily in a courteous and professional manner, addressing questions and directing calls promptly.
- Serve as part of the care team in developing person‑centered diabetes plans and implement workflows supporting the Patient Centered Medical Home model.
- Advocate for and support technology‑enabled diabetes education and care in individual and population health diabetes services.
- Attend and complete continuing education related to this position and other topics as deemed necessary by MHSI.
- Collaborate with community partners to establish and maintain ongoing support options.
- Participate in all Risk Management activities as deemed appropriate.
- Perform additional duties or responsibilities as deemed necessary.
- Associate’s Degree in Nursing required or Bachelor of Science in Nursing.
- RN with one year or more related experience preferred.
- Proficiency with Excel spreadsheets, graphs, and Word documents; ability to learn and use electronic medical record systems.
- Current licensure as a registered nurse, required by Arkansas State Nursing Board.
- BLS certification required.
- Willingness to attend and/or complete continuing education related to this position and other topics deemed necessary by MHSI.
- Detail oriented, positive attitude, ability to take direction and work well with a team, and self‑motivated.
- Excellent verbal and written communication skills.
- Ability to work with a diverse patient population and provide exceptional customer service.
- Ability to work in a constant state of alertness and safely.
The Health Coach reports to and is supervised by the Director of Health Education.
