Nurse Practitioner

This role offers strong work-life balance with flexible scheduling. Providers can work between 8am–8pm across a 7-day week
, with no on-call, no weekends, and no holidays required.

About the Role

As part of Optum Home & Community Care’s House Calls program, you’ll deliver preventive care directly in patients’ homes. This is a non-prescribing, field-based position focused on improving outcomes through education, assessments, and care coordination.

What You’ll Do

• Conduct in-home health assessments (history, vitals, physical exam)
• Perform screenings and point-of-care testing as appropriate
• Document diagnoses to support care planning
• Communicate findings with primary care providers
• Identify urgent issues and respond appropriately
• Educate patients on conditions, medications, and compliance
• Address social needs and connect patients to resources

Required Qualifications

• NP (ANCC/AANP) or PA (NCCPA) certification
• Active, unrestricted state license (or ability to obtain)
• Valid driver’s license + reliable transportation
• Ability to complete in-home visits in varied environments

Preferred

• 1+ year of clinical experience (geriatrics, family, or home health)
• Comfort with EMR systems/technology
• Experience working with Medicare or senior populations
