In-Home Nurse Practitioner or Physician Assistant (Part Time) - Portland, OR

Job Overview

Signify Health, part of CVS Health®, is seeking a Part‑Time Clinician (Nurse Practitioner or Physician Assistant) to provide In‑Home Health Evaluations and related services in patients’ private residences.

• Conduct in‑home health evaluations lasting approximately 45 minutes, including medication and medical history review, physical assessment, and any diagnostic tests ordered by the health plan (e.g., diabetic eye exams, spirometry).
• Build trusted relationships with patients and connect them to appropriate care when needed.
• Participate in a national program that aims to visit millions of people nationwide, providing connections to the right care for individual needs.
• Travel locally and within licensed states as required, and conduct virtual visits when indicated.
• Provide all Signify Health services as directed, including diagnostic and preventive services such as spirometry, DEE, bone density screening, and others.
• Follow a clinical workflow using an iPad; not required to prescribe medication, order lab tests, or alter current treatment regimens.

• Active, unrestricted license(s) in coverage area(s).
• Board certification (required).
• Multi‑state license(s) (preferred).

Time Type: Part Time

Pay Range: $41.85 – $90.13 per hour (actual offer based on experience, education, geography, and other factors).
