Signify Health, part of CVS Health, is seeking a Part-Time Clinician (Nurse Practitioner or Physician Assistant) to perform in-home health evaluations in patients' residences. Each visit lasts about 45 minutes and includes medication and medical history review, physical assessment, and any required tests ordered by the health plan.
You will build trust with patients, connect them to appropriate care, and travel locally to licensed states; some visits may be virtual.
