JOB SUMMARY

The Nurse Practitioner (NP) or Physician Assistant (PA) is responsible for delivering high-quality, patient-centered primary care services across the lifespan of patients. This includes conducting comprehensive assessments, diagnosing and managing acute and chronic conditions, prescribing medications and treatments and providing health education and preventive care.

Working collaboratively with physicians, specialists and other members of the care team, the provider plays a vital role in supporting a coordinated and coordinated care approach to patient wellness.

The NP/PA must function within the defined scope of practice as legislated by the State of Oregon and adhere to all regulatory, licensing and professional standards.

Responsibilities Include:
  • Performing physical exams, diagnostic evaluations and screenings
  • Diagnosing and managing acute illnesses, chronic conditions and minor injuries
  • Prescribing medications, therapies and other medical interventions as appropriate
  • Developing and managing individualized care plans in collaboration with patients and the care team
  • Educating patients and families on health maintenance, disease prevention and treatment options
  • Coordinating referrals to specialists and other healthcare services as needed
  • Documenting care accurately and thoroughly in the electronic health record (EHR)
  • Participating in quality improvement initiatives and clinical team meetings
  • Maintaining licensure, certifications and continuing education as required by law and professional standards
  • Supporting a positive, team-based care environment aligned with the mission, vision and values of the organization

Job Description QUALIFICATIONS
  • Graduate of an accredited Nurse Practitioner (NP) or Physician Assistant (PA) program required
  • Current and unrestricted licensure as a Nurse Practitioner or Physician Assistant in the State of Oregon required
  • National board certification as a Nurse Practitioner or Physician Assistant (e.g., AANP, ANCC, NCCPA) required
  • Current BLS/CPR certification required
  • Commitment to completing all required annual trainings and organizational competencies
  • Must maintain continuing medical education (CME) in accordance with state licensure and certification requirements
  • Strong clinical assessment, diagnostic and treatment planning skills
  • Excellent communication, collaboration and patient education abilities
  • Demonstrated ability to work independently and as part of a multidisciplinary healthcare team
  • Experience with electronic health records (EHR) and comfortable navigating technology in a clinical setting preferred
  • Demonstrated ability to provide compassionate, patient-centered care across diverse populations
  • Ability to meet the physical and functional requirements outlined in the job description
  • High School Diploma or equivalent required
  • Must successfully pass a criminal background check, drug screen and reference checks
