The RN Care Coordinator - ED is responsible for managing patient transitions in acute care and emergency settings, ensuring quality clinical coordination and care management. This role is ideal for experienced nurses who excel in patient assessment and interdisciplinary collaboration.

• Coordinate post-discharge patient care needs for timely and effective discharges.
• Independently complete and document thorough patient assessments.
• Develop mutually agreeable plans of care with patients, families, and the health care team.
• Facilitate resolution of issues that present barriers to safe transfers.
• Ensure ongoing contact with interdisciplinary staff for safe patient transitions.
• Collaborate with the health care team to meet regulatory and compliance standards.
• Perform duties as requested.

• Associate's Degree (ADN) required

• Leadership experience preferred

• Current RN License in Oregon
