Preschool Inclusion Developmental Disabilities Case Manager

Responsibilities

• Provides comprehensive, intensive case management and complex service coordination for toddlers accessing pre-school.
• Utilizes advanced assessment skills and in-depth knowledge of multiple interconnected systems and childhood development to mitigate complex support needs.
• Coordinates services, ensures child safety, and facilitates transition to an educational setting.
• Conducts comprehensive, advanced assessments to determine complex support needs of children identified by the preschool provider.
• Develops, implements, and monitors support plans that address unique needs, choices, strengths, and risks of the child for preschool access.
• Monitors and documents behavior services compliance with approved models.
• Identifies gaps in community services and recommends cost-effective solutions.

• Knowledge of the public service system for developmental disabilities services in Oregon
• A bachelor’s degree in behavioral science, social science, or a closely related field, OR a bachelor’s degree in any field and one year of Human services related experience, OR an associate’s degree in a behavioral science, social science, or a closely related field and two (2) years of human services related experience, OR three (3) years of human services related experience.
• Must have a valid driver license
• Must pass a criminal background check
