Mercy Health in St. Charles is seeking a Social Worker to provide direct patient care under supervision in accordance with regulations and hospital policies.
The role supports discharge planning, psychosocial assessments, and collaboration with the care team to enhance patient outcomes. The successful candidate will have a Bachelor’s in Social Work (required), a Master’s in Social Work or Social Administration preferred, and licensure in practice state (within six months of hire).
