Ambulatory Clinical Pharmacy Specialist
Join our Shields team today! Shields Health Solutions is redefining patient care through health‑system integrated specialty pharmacy. We’re hiring an Ambulatory Clinical Pharmacy Specialist who will play a hands‑on role in elevating patient care at a nationally recognized Academic Medical Center. In this dynamic, patient‑centered position, you’ll build meaningful relationships with patients and providers while delivering high‑quality medication management. You’ll work remotely, collaborating directly with care teams and practicing under a Collaborative Practice Agreement (CPA) to conduct virtual patient assessments, order and monitor evidence‑based treatment plans, optimize therapy, and resolve medication‑related problems.
Location: Remote (Role supports a clinic in California)
Salary: Employees performing duties in California can expect a salary between $145,000 and $180,000 (hourly equivalent: approximately $69.71–$85.54 per hour, based on a 40‑hour workweek). Employees in other states can expect a salary between $127,000 and $180,000 (hourly equivalent: approximately $61.06–$85.54 per hour). Bonus is paid pursuant to the terms of any applicable bonus plan. Salary depends on experience, location and other factors allowed by law.

- Provide comprehensive medication management, including reconciliation, dose optimization, adherence support, and drug‑level assessments aligned with evidence‑based standards.
- Partner with physicians, specialists, and interdisciplinary teams to order individualized care plans under a CPA and promote seamless medication coordination.
- Monitor therapy effectiveness and safety, identify optimization opportunities and intervene as appropriate.
- Provide recommendations to operations by supporting teammates as needed with complex prior authorizations, improving workflows, educating patients, and maintaining ongoing clinical development.
- Hold a Doctor of Pharmacy (PharmD)
- Must be a licensed pharmacist in California
- Candidate must meet one of the following qualifications:
- Completion of a PGY2 Ambulatory Care Residency
- Completion of a PGY1 Residency and at least 1 year of ambulatory practice under a Collaborative Practice Agreement (CPA)
- At least 3 years of ambulatory practice with disease state management
- Strong communication, organization, and tech skills
- Empathy, integrity, and a team‑first mindset
