ABOUT

THE ROLE

Schedule: Monday-Friday 8:00am-5:00pm

Territories: Salt Lake City Area, Murray, Payson

HOSPICE CHAPLAIN ROLE
  • The Hospice Chaplain is responsible for the provision of spiritual care services to patients and families/caregivers of Sage Hospice and Palliative Care either directly or through coordination of care with other counselors.

ROLE REQUIREMENTS
  • Associate’s degree in divinity, theology, or biblical studies or certification as an ordained minister.
  • One (1) year of experience in hospice or healthcare.
  • In Georgia only: documented completion of Clinical Pastoral Education (CPE), per state requirements.
  • Experience with patients, families/caregivers through death or dying.
  • Must have reliable transportation, proof of current automobile insurance and a valid driver’s license.
BEREAVEMENT COORDINATOR ROLE
  • As a Bereavement Coordinator, you're responsible for planning, implementing and maintaining a bereavement program to meet the needs of family members and/or caregivers for up to one (1) year following the transition of a hospice patient.
BEREAVEMENT COORDINATOR REQUIRED EDUCATION/

EXPERIENCE:
  • Bachelor’s degree in social work or theology or related experience.
  • One (1) year experience in clinical grief counseling and response with patients, families, or caregivers through death/dying issues.
  • Must have reliable transportation, proof of current automobile insurance and a valid driver’s license.
PREFERRED
  • 1-2 years of experience as a Chaplain
