Provides certified nursing assistant services to assigned residents in accordance with care plans, facility policies and procedures, and at the direction of supervisor(s).
Responsibilities- Assist residents with activities of daily living, including bathing, grooming, dressing, feeding, and toileting.
- Assist with lifting, turning, and transferring residents between beds, chairs, wheelchairs, and other equipment.
- Coordinate dining room services at assigned mealtimes, set up and clean up, deliver meal trays, provide feeding assistance, and document intake.
- Deliver nutritional supplements and record intake.
- Weigh residents, record weight, and report changes to nurse and supervisor.
- Monitor skin condition and report pressure areas, skin breakdown, or tears to nurse and supervisor.
- Assist nursing staff with the toileting program.
- Complete flow sheets daily to indicate that tasks were performed.
- Document resident progress as directed.
- Notify nurse and supervisor of any impediments to performing work (e.g., resident refusal or reassignment).
- Report changes in residents’ condition to nurse and supervisor.
- Promote a culture of compliance by adhering to all facility policies, procedures, and regulations.
- Maintain confidentiality of protected health information.
- Report non‑compliance, retaliation, abuse, neglect, or hazards related to care to appropriate personnel.
- Participate in life‑safety and emergency drills and fulfill responsibilities during activation of the emergency plan.
- Report work‑related injuries or illnesses promptly.
- Follow infection‑control policies and procedures.
- Complete all assigned training and competency evaluations.
- Attend care‑plan meetings for assigned residents.
- Participate in QAPI and performance improvement activities as required.
- Accept CNA assignments as staffing needs require.
- Use proper lifting techniques when delivering care.
- Certified Nursing Assistant in good standing with the state (preferred, not required).
- High school diploma or equivalent.
- Current CPR/BLS certification.
- Current Food Handler Permit.
- No disciplinary action in effect against certification.
- At least one year of experience as a certified nursing assistant in long‑term care (preferred, not required).
- Ability to read, write, speak, and understand English.
- Supportive team member with strong interpersonal skills and patience.
- Capability to make independent decisions when warranted.
- Proficiency with common physical tasks such as bending, stooping, kneeling, and lifting at least 35 pounds.
- Ability to work beyond normal hours, including weekends and holidays when necessary.
- Basic computer skills for electronic medical record systems.
- Ability to manage conflict and handle difficult residents and staff professionally.
Employment is contingent upon adherence to all standards, policies, and procedures of the facility. Violations, including failure to report violations, may result in disciplinary action up to termination.
