Our goal is to keep mixed‑Medicaid eligible families visiting the doctor together and to provide continuity of care as individuals fall in and out of eligibility.

Full time roles are eligible for additional variable compensation plan.

We are seeking a compassionate and skilled Physician Assistant (PA) or Nurse Practitioner (NP) to join our dynamic team and provide exceptional primary care to our patients.

• Independently delivers pediatric, adolescent, adult, and geriatric healthcare and patient management.
• Orders and interprets diagnostic tests.
• Prescribes medications and in some cases, dispenses.
• Educates patients and/or families about preventive care, medical issues, and use of prescribed medical treatments and/or medications.
• Monitors the effectiveness of interventions.
• Records medical histories and conducts physical exams.
• Protects patients and employees by adhering to infection‑control policies and protocols.
• Operates basic medical equipment and instruments.
• Performs minor procedures such as suturing, casting and other minor procedures.
• Champions new patient acquisition and growth by attending events in the community as needed.
• Adheres to all HIPAA protocols, clinical guidelines, and company policies and procedures.
• Participates in team meetings, education conferences, and special projects as required.
• Charts ICD and CPT codes for billing purposes and closes charts in a timely manner.

• Graduation from an accredited Physician Assistant or Nurse Practitioner program.
• Current state licensure or eligibility for licensure to practice in the state.
• Minimum of 2 years of physician assistant or nurse practitioner experience including pediatric and adult populations.
• Current National Certification and Drug Enforcement Agency (DEA) Certification required.
• Exceptional written and verbal communication skills.
• Proficiency in Spanish and English required.
• HCC or RAF knowledge is desired.

• Generally, an indoor setting.
• Will be using a computer frequently.
• Varying schedule to include evenings, holidays and extended hours as business dictates.
• Occasionally exposed to heat/cold while performing duties.
