Position Announcement

As a Case Manager with the UVU CARE Hub, you’ll have the opportunity to make a meaningful difference in the lives of students navigating challenges related to food, housing, health, and safety. You’ll shape and lead essential case‑management services that help students stay focused on their education by ensuring they have access to the resources and support they need. This role offers the chance to design processes, guide students through complex situations, and strengthen the university’s basic‑needs infrastructure in ways that directly support student success.

Summary

of Responsibilities
  • Manage a comprehensive caseload of student clients, assessing their needs and developing individualized plans to address housing, healthcare, and other support services. Conduct intake interviews to determine clients’ requirements and eligibility for services. Provide personalized case management, including follow‑up and ongoing support, to ensure clients achieve their goals.
  • Design and implement case‑management policies and procedures that align with the CARE Hub’s mission and goals. Develop schedules and routines to ensure clients receive consistent, comprehensive services. Coordinate with on‑ and off‑campus service providers to arrange necessary resources and support for clients.
  • Establish and maintain relationships with community organizations, campus entities, housing authorities, and other relevant stakeholders. Act as a liaison between clients and external service providers, ensuring effective communication and coordination. Conduct outreach and awareness campaigns to promote on‑campus case‑management services. Advocate on the client’s behalf to community organizations and campus entities.
  • Maintain accurate and up‑to‑date records of client interactions, service plans, and outcomes. Prepare and submit detailed reports on program activities, client progress, and performance metrics. Ensure compliance with all relevant regulations and guidelines related to case‑management services. Evaluate clients’ progress periodically and make adjustments as needed to improve outcomes.
  • Perform other job‑related duties as assigned.
Qualifications / Licenses / Certifications
  • Bachelor’s in social work, communication, community health, or related field; OR a combination of education and experience totaling four years.
  • Demonstrated ability to establish and maintain collaborative working relationships.
  • Demonstrated experience working effectively on a team.
  • Demonstrated commitment to public service.
  • Experience working with a variety of individuals.
Preferred Qualifications
  • An MSW or related degree.
Knowledge / Skills / Abilities Knowledge
  • Knowledge of case‑management principles, practices, and techniques.
  • Understanding of relevant federal, state, and local regulations related to housing and support services.
Skills
  • Strong organizational, verbal, and written communication skills.
  • Proficiency in database management and assessment tools.
  • Strong problem‑solving and decision‑making skills.
Abilities
  • Ability to effectively manage a moderate caseload and provide individualized support to clients.
  • Ability to establish and maintain strong relationships with community partners and stakeholders.
  • Ability to educate clients and community members about available services and resources.
  • Ability to plan, coordinate, and evaluate case‑management programs.
