The Case Aide assists staff in non-technical services such as making outreach to referred families to screen for FFT and BCC eligibility, monitoring of program trackers which provide client census and families served data, assist with collection of evaluation forms, surveys, and assessments, and routine clerical tasks.
We are looking for a Case Aide who must have:- High School Diploma required;
Associates of Arts preferred. - One to two years of administrative/clerical office experience.
- One year of general clinical experience, preferably in Human or social service setting.
- Monitor and update referred client census including tracking incoming referrals, current cases, client case files, and discharged outcomes.
- Monitor and input Behavioral Health client satisfaction survey responses upon completion of FFT and BCC services.
- Analyze referred client outcome data, including obtaining total number of families referred, total number of families who accepted services, and percentage of successful and unsuccessful closures for monthly stakeholder monitoring report.
- Monitor and update in office safety items and ensure that all office licenses and certifications are valid.
- Monitor and maintain office essentials as needed including ordering, purchasing, and obtaining office supplies.
- Participate in all training required by BAYS
- Performs all other related duties as assigned.
- Assist in gathering referral information from referral source
- Provide general administrative support to team.
- Maintain the referral tracker on a daily basis to prepare for monthly data reporting.
- Serve as the on-site Safety Representative
- Conduct initial outreach to referred families to explain BAYS Family Connections Program, assess their eligibility for FFT and BCC services including completing eligibility forms, send client outreach letters to unresponsive families, and follow-up with referral sources.
