LeeKO Home Health LLC provides exceptional home health services, helping patients regain independence and improve quality of life. We are seeking a skilled Occupational Therapist (OT) and Occupational Therapist Assistant (COTA) to join our team.
Responsibilities OT:
- Deliver in-home occupational therapy per physician orders.
- Develop treatment plans to improve daily living skills.
- Educate patients and caregivers on exercises and mobility.
- Document visits in Synergy EMR.
- Collaborate with interdisciplinary team members.
- Support OT in patient care plans.
- Document progress accurately.
- Document visits in Synergy EMR.
Qualifications OT:
- 1+ year clinical experience (home health preferred).
- Strong communication,documentation, and organizational skills.
