Care Management Technician
Position Summary:
Care Management Technician focused on supporting the care management team in achieving departmental goals and objectives of patient transition throughout.

- Educate patients and families on the roles of the care management team, bridging communications with other health care team members, and ensuring timely transitions to the next level of care.
- Assist with Care Management requests, including arranging nursing home returns, contacting SNFs, ALFs and rehabilitation centers for bed availability, identifying community resource availability, enrolling patients in assistance programs, referring to community agencies, arranging transportation, obtaining authorization numbers from payors, and facilitating durable medical equipment.
- Issue important Medicare communications such as IMM letters, MOON notifications, ABNs and HINNs as appropriate.
- Support overall office functions: mail management, phone coverage, safe office operations/maintenance of office equipment, filing, statistics and reports, databases and spreadsheets, scheduling meetings, and implementing process improvements.
- Provide time‑keeping support via departmental procedures and data entry into the corporate time management system.
- Participate in quality and safety initiatives to improve practice.
- Prioritize workload and maintain regular, punctual attendance consistent with Orlando Health policies, the ADA, FMLA and other applicable standards.
- High school graduate or equivalent.
- None required for licensure or certification.
- Two (2) years’ experience working with customers and/or patients, including one year in a health‑related field.
- Working knowledge of Microsoft Office programs (Word, Excel, Outlook, PowerPoint).
