Acute Home Healthcare is looking for a Medical Social Worker (MSW) in Osceola county to join our dynamic team.

Candidate Must Have
- Have a Master's degree from a school accredited by the Council on Social Work Education
- Has a bachelor's degree in social work, psychology, sociology or other field related to social work
- One year of Home Health experience
- Ability to exercise initiative and independent judgment
- Must be a team player
- Level 2 Background screening
Responsibilities
- Serves as a resource person to employees, patients, physicians and other allied healthcare providers
- Participates in the development of the plan of care
- Prepares clinical and progress notes
- Assists physician and other team members in understanding the significant social and emotional factors related to health problems
- Works with the family
- Participates in discharge planning
- Documents all patient/family services provided as required by Agency policy
- Plans/coordinates all social services within the Agency
- Maintains contact with community support groups and provides professional expertise as required
- Maintains current knowledge of Medicare Conditions of Participation for social work services
- Immediately reports any accident, incident, lost articles or unusual occurrence to Director of Clinical Services/Nursing Supervisor
- Participates in patient care conferences
- Participates in the coordination of Agency's services with the services of other community resources; uses community resources
- Serves as a resource person to employees, patients, physicians and other allied healthcare providers
- Other duties assigned by Director of Clinical Services/Nursing Supervisor
This is a Per Diem position
Job Types: Full-time, Part-time
Pay: $85.00 per hour
