Job Description:

Other duties as assigned. Develops and delivers educational materials and training programs for Care Management. Collaborates with Care Management leadership to identify educational gaps and create campus-specific educational plans. Conducts New Hire Care Management training to ensure new care managers receive necessary education and tools. Mentors and trains care management team members to enhance workflow processes and service quality. Supports performance improvement activities through problem identification, assessment, data gathering, change implementation, and evaluation of outcomes.

Participates in Process Improvement Initiatives and ensures practice changes are implemented. Audits care management compliance with regulatory regulations and documentation standards. Reviews and revises Care Management Policies, SOPs, and Job Description s as needed. Identifies community resources and coordinates education opportunities for Care Management staff.
