- Provides referrals for patients within the agency and community.
- Works directly with patients, family members, physicians, and community providers to ensure collaborative and continuous care.
- Identifies and intervenes for suspected abuse, including sensitivity to age-specific concerns.
- Participates in multidisciplinary treatment team meetings, providing clinical input.
- Formulates discharge plans in collaboration with the multidisciplinary treatment team, the patient, and the family, ensuring outpatient appointments are scheduled upon inpatient discharge.
- Demonstrates working knowledge of the involuntary commitment process.
- Conducts comprehensive assessments to include risk assessment and establishes therapy treatment objectives.
- Facilitates individual, family, and group therapy sessions.
- Evaluates patient data; develops and implements treatment plans.
- Documents therapy observations and maintains records, adhering to confidentiality protocols.
- Evaluates patient progress and therapy intervention effectiveness
- Other duties as assigned.
