ORTHO/NEURO NURSE NAVIGATOR Jewett Orthopedics Institute OHJOI PATIENT NAVIGATORS Full Time Position Summary
This role enhances the patient’s experience by providing seamless navigation from pre-procedure scheduling and throughout the care continuum. The Navigator collaborates with all members of the interdisciplinary team to coordinate and consistently manage patient care by serving as a point of contact for patients and families. The Navigator systematically and continually performs the functions of assessing, planning, implementing, and evaluating care according to the nursing process.
Education/ Training
An Associate of Science in Nursing (ASN) is required. A Bachelor of Science in Nursing (BSN) is preferred.
Licensure / Certification- Maintains license as an RN in the state of Florida.
- Maintains current BLS/healthcare provider certification.
Minimum of two (2) years of clinical experience in acute care is required. Ortho/Neuro experience is preferred.
Essential Functions- Coordinates patient care through the continuum. Assists with timely access to care, coordinates/facilitates appointments and services, and encourages adherence to treatment plans, protocols, and follow up care.
- Assesses patients’ current and future needs, provides access to resources (internal and external), and makes appropriate referrals as needed.
- Provides education and information to patients and families related to the healthcare system, multidisciplinary team member roles, plan of care, and available resources.
- Establishes and maintains close collaboration with physicians and multidisciplinary team members to ensure seamless patient care and care coordination.
- Tracks determined program metrics and quality indicators. Documents patient interactions and progression, and communicates navigation activities to physicians, the care team, and leadership.
- Collaborates with the clinical team to obtain or develop patient and family educational materials and resources.
- Demonstrates knowledge, skill, and coordination to provide nursing care and guidance to patients from screening to post-operative care and follow up.
- Demonstrates effective communication with patients, families, peers, members of the multidisciplinary healthcare team, and representatives of community organizations and resources.
- Maintains reasonably regular, punctual attendance consistent with Orlando Health policies, the ADA, FMLA and other federal, state and local standards.
