Position Summary
We are seeking a Registered Nurse Care Coordinator to assess and coordinate care for patients and families, delivering palliative and supportive services as part of an interdisciplinary team.
Key Responsibilities- Assess patient and family needs using the nursing process and develop the Plan of Care with the Interdisciplinary Group.
- Provide palliative and supportive care at home and in the community.
- Document assessments and care plans in the EMR and communicate with the IDT using SBAR.
- Coordinate in‑person visits when needed and assist patients in navigating services.
- Perform initial and semi‑annual assessments for all company services.
- Educate patients, families, and caregivers about disease progression, prevention, and end‑of‑life issues.
- Supervise Community Health Workers and Home Health Aides during patient visits.
- Participate in daily IDT collaborative meetings and record concise reports.
- Maintain compliance with state, federal, and company policies, including fraud prevention and confidentiality.
- Respond to emergency situations and support continuity of services during disasters.
- Perform other related duties as assigned.
- Current Registered Nurse license in the state of employment.
- Minimum one year of nursing experience; hospice or hospital experience preferred.
- Completion of Alzheimer’s Disease and Related Dementias training (Florida) or equivalent.
- Experience with an EMR/EHR system.
- Valid driver’s license and automobile insurance; reliable transportation for home visits.
- Ability to use visual and auditory equipment.
- Excellent verbal and written communication in English.
- Physical ability to lift patients, bend, push/pull, kneel, and meet essential job functions.
- Active Basic Life Support certification from the American Heart Association or Red Cross; additional BLS may be required.
- Mobile Driver skills with the ability to travel for patient visits.
$60,058.27 – $90,087.92 annually. The position requires consent to drug and alcohol testing and ongoing background screening in accordance with state regulations.
