Licensed Practical Nurse (LPN) – Full‑time, in‑home nursing care to pediatric patients. We offer comprehensive training and 24/7 support from leaders.
Key Responsibilities- Accountable for providing care that is based on licensure/certification within the scope of practice.
- Maintain an updated clinical record on each patient at all times, meeting required deadlines for documentation.
- Evaluate the effectiveness of nursing service to the patient and family on an ongoing basis.
- Consult with physicians concerning alterations of patient care plans, document changes and report all changes in care to the nurse supervisor.
- Support FFHC’s mission by mentoring and/or coaching new employees during patient‑specific orientations.
- Educate patients and their caregivers on topics related to patients’ care needs.
- Participate in FFHC Quality Assurance, Performance Improvement program (QAPI) as requested.
- Continually strive to improve nursing care skills.
- Attend and/or complete all mandatory training/ in‑services as assigned.
- Submit required personnel file documentation in a timely manner.
- Adhere to FFHC compliance and ethics expectations.
- Maintain patient and family confidentiality.
- Maintain professional, supportive, and responsive interpersonal communication skills.
- Perform other duties as assigned.
- LPN/LVN Nursing Degree.
- Unencumbered LPN/LVN License in the state of employment or multi‑state license through the Enhanced Nurse Licensure Compact where recognized.
- 1 year of demonstrated experience.
- Valid driver license with proof of insurance in applicable states.
- Clear background screening.
- Working knowledge of Nurse Practice Acts.
- Valid and current CPR card/certificate with in‑person instructor led hands‑on skills check.
- Clear Hep B Screening in applicable states.
- Clear TB Screening in applicable states.
- State‑specific requirements and additional background screening as noted for Florida, Illinois, Pennsylvania, Texas.
- Ability to read and communicate in English both verbally and in writing.
- Work performed in the patient’s home; not substantially exposed to adverse environmental conditions.
- Training may be performed in an office setting.
- Must be able to lift up to 35 pounds (35 lbs) without an assisted lifting device.
- Frequent use of office machines to include telephone, computer, and printer.

- Work schedule may include varying shift lengths (8, 10, or 12 hours) and may require availability on any day of the week, Sunday through Saturday.
