Position: Telephonic Nurse Practitioner (Part-Time) - Florida License Required

Role Summary

Telephonic After‑Hours Call – Evenings, Nights & Rotating Weekends Required. Part‑time (20 hours per week). The National On‑Call Advanced Practice Clinician (APC) is responsible for providing telephonic/telehealth care and direction to patients, caregivers and facility staff providing 24/7 coverage including holidays.

Scheduling
  • Flexibility and the ability to adapt are required as you will cross‑cover multiple markets and teams.
  • Coverage expectations:
    • 24/7 coverage
    • Minimum commitment of 20 hours per week
    • Every other weekend coverage between 8–12 hour shifts covering both day and night as needed based on business needs.
    • Employees are expected to work or have approved PTO for 26 weekends a year. Each employee may have up to 6 weekend shifts a year for PTO.
    • Unapproved time away / unpaid time off will require additional weekend shifts to be added to your schedule.
  • Holidays are required for all APCs on a rotation basis. Holiday scheduling is arranged at the beginning of the year for advanced planning.
  • Holiday coverage is provided from 5 pm, the end of the last business day, to 8 am of the next business day.
Primary Responsibilities
  • Be available on the provided telephonic platform to take and place calls to coordinate and manage care for members, caregivers, facilities, hospitals, primary care providers, and Optum field colleagues.
    • Use video platform as clinically indicated.
    • Work from a secure location that protects patient privacy.
  • Utilize EMR proficiently to provide acute care during all shifts and holiday hours.
  • Care Delivery:
    • Deliver cost‑effective, quality care to members.
    • Manage medical and behavioral, chronic, and acute conditions in collaboration with physicians or specialty providers.
    • Perform comprehensive assessments and document findings concisely in compliance with CMS regulations.
    • Ensure encounters are documented to support diagnoses and quality elements.
    • Adopt evidence‑based practice guidelines and complete mandatory education and My Learning training requirements.
  • Care Coordination:
    • Coordinate care as members transition through different levels and settings.
    • Monitor needs of members and families and facilitate adjustments to the plan of care.
    • Review orders and interventions for appropriateness and response to treatment.
    • Conduct advanced care planning conversations with members and families.
    • Evaluate the plan of care for cost effectiveness while meeting the needs of members, families, and providers.

    • Meet shift scheduling requirements and attendance expectations.

    • Check Optum email intermittently for trainings, communications, and scheduling.
Required Qualifications
  • Education:

    NP – Graduate of an accredited Master of Science Nursing or Doctor of Nursing Practice program.
  • Active and unrestricted license in the state of residence and in the State of Florida.
  • Ability to obtain a collaborative practice agreement if applicable.
  • For jurisdictions that allow autonomous practice, obtain approval to practice autonomously or without formal supervision within the required timelines.
  • Active Nurse Practitioner certification through a national board (e.g., American Academy of Nurse Practitioners or American Nurses Credentialing Center). Certification must be in one of the following:
    Family, Adult, Gerontology, Adult‑Gerontology Acute Care.
  • Current, active DEA licensure/prescriptive authority or ability to obtain it per state regulations.
  • Ability to work scheduled shifts in accordance with scheduling policies.
