Pay Rates: $30 – $35.

• Provide comprehensive nursing services, adhering to Family First Homecare policies, state Nurse Practice Acts, and applicable regulations.
• Maintain updated clinical records for each patient and meet documentation deadlines.
• Evaluate nursing service effectiveness and consult with physicians on changes to patient care plans.
• Document changes in care and report all changes to the nurse supervisor.
• Mentor and coach new employees during patient‑specific orientations.
• Educate patients and caregivers about care needs.
• Participate in FFHC Quality Assurance and Performance Improvement programs as requested.
• Continuously strive to improve nursing care skills and complete mandatory training/in‑services.
• Submit required personnel file documentation on time and adhere to compliance and ethics expectations.
• Maintain patient and family confidentiality and maintain professional, supportive communication skills.
• Perform other duties as assigned.

• LPN/LVN Nursing Degree and unencumbered LPN/LVN license (or multi‑state license via Enhanced Nurse Licensure Compact).
• At least 1 year of demonstrated experience.
• Valid driver license with proof of insurance in applicable states.
• Clear background screening.
• Working knowledge of Nurse Practice Acts.
• Valid and current CPR card and in‑person instructor‑led hands‑on skills check.
• Clear Hep B and TB screening in applicable states.
• Ability to read and communicate in English verbally and in writing.
• Additional background screening for child abuse and FBI check in Pennsylvania.
• Completed TB screening with 2‑step PPD or proof of 2‑step PPD within last year (Pennsylvania).

Work is performed in patients' homes. Shifts may include 8, 10, or 12 hours and may require availability any day of the week. Must lift up to 35 pounds without an assisted lifting device. Frequent use of office machines such as telephone, computer, and printer.
