Position SummaryOlive Health is seeking a Part‑Time Nurse Practitioner to provide comprehensive primary care services to patients in assisted living facilities, group homes, and private residences throughout the Orlando area. This hybrid role combines field‑based patient visits with remote administrative responsibilities, offering flexibility while maintaining a strong focus on direct patient care.
The ideal candidate is highly motivated, patient‑centered, and comfortable practicing independently while collaborating closely with physicians, home health providers, specialists, and community partners.
Key Responsibilities- Conduct comprehensive patient assessments and develop individualized treatment plans.
- Provide primary care services, including preventive care, chronic disease management, and acute care treatment.
- Manage and monitor patients with complex medical conditions.
- Order, interpret, and review laboratory tests, diagnostic studies, and imaging results.
- Prescribe medications and treatments in accordance with Florida regulations and evidence‑based guidelines.
- Coordinate care with home health agencies, specialists, hospitals, and community providers.
- Perform post‑hospitalization and transitional care follow‑up visits.
- Maintain accurate and timely documentation within the electronic health record (EHR).
- Participate in interdisciplinary care planning and case management discussions.
- Support quality improvement initiatives and patient outcome goals.
- Master’s or Doctorate degree from an accredited Nurse Practitioner program.
- National board certification as a Family Nurse Practitioner (FNP), Adult‑Gerontology Nurse Practitioner (AGNP), or related specialty.
- Minimum of 1 year of Nurse Practitioner experience preferred; experience in primary care, geriatrics, home‑based care, assisted living, or post‑acute care strongly preferred.
- Strong clinical assessment, diagnostic, and care coordination skills.
- Excellent communication and interpersonal abilities.
- Reliable transportation and willingness to travel throughout the Orlando service area.
