Position Summary
ORTHO/NEURO NURSE NAVIGATOR – South Lake Hospital, serving the Jewett Orthopedic Institute. Full‑time. This role supports the orthopedic patient population at South Lake Hospital, providing Nurse Navigator services for pre‑ and postoperative follow‑up with total joint knee, hip, and shoulder patients as part of the growing Varma Surgical Group. The position follows the established structure of the existing Ortho/Neuro Nurse Navigator role across central, east, and west locations.
Education& Training
- Bachelor of Science in Nursing (BSN) – required.
- MSN – preferred.
- Active RN license in the state of Florida.
- Current Basic Life Support (BLS) / Healthcare Provider certification.
- Minimum of two (2) years of clinical experience in acute care – required.
- Ortho/Neuro experience – preferred.
- Coordinates patient care through the continuum, ensuring timely access to care, arranging appointments and services, and encouraging adherence to treatment plans and follow‑up care.
- Assesses patients’ current and future needs, provides access to resources (internal and external), and makes referrals as needed.
- Provides education and information to patients and families about the healthcare system, multidisciplinary team roles, care plans, and available resources.
- Establishes and maintains close collaboration with physicians and multidisciplinary team members to ensure seamless patient care and coordination.
- Tracks program metrics and quality indicators, documents patient interactions and progression, and communicates navigation activities to physicians, the care team, and leadership.
- Collaborates with the clinical team to develop or obtain patient and family educational materials and resources.
- Demonstrates nursing care and guidance to patients from screening to post‑operative care and follow‑up.
- Communicates effectively with patients, families, peers, multidisciplinary healthcare team members, and community organizations and resource representatives.
- Maintains reasonably regular, punctual attendance consistent with Orlando Health policies, the ADA, FMLA, and other federal, state and local standards.
- Maintains compliance with all Orlando Health policies and procedures.
