Lighthouse Home Health is a locally owned agency delivering high-quality, Medicare-certified skilled home health services to the community. The organization focuses on safe transitions home, improved patient outcomes, and compassionate, personalized care for individuals in need of support. Clinical services, upon licensure, will include skilled nursing for medication management, wound care, and chronic disease monitoring. Additional services will encompass physical therapy for strength, balance, and mobility;
occupational therapy to support daily living safety and independence; speech therapy for swallowing, cognition, and communication; and home health aide services for personal care under RN supervision. Team members join a growing agency committed to collaborative, interdisciplinary care in the home setting.
The Home Health Nurse is a part-time, on-site role based in Orlando, FL. This position provides skilled nursing care in the home, including medication administration and management, wound assessment and treatment, and monitoring of chronic conditions. Daily tasks involve performing comprehensive patient assessments, developing and updating individualized care plans, educating patients and caregivers on disease management and safety, and coordinating services with therapy and home health aide staff.
The Home Health Nurse will document all care in accordance with regulatory requirements, communicate changes in patient status to the healthcare team, and contribute to safe transitions between care settings. The role requires reliable travel to patient homes within the service area and adherence to agency policies and clinical standards.
- Clinical expertise in Nursing and Medicine to provide safe, evidence-based care in the home setting.
- Demonstrated experience in Home Care and Patient Care, including in-home assessments, care planning, and patient education.
- Proficiency in Wound Care, including dressing changes, infection prevention, and monitoring of healing.
- Active Registered Nurse (RN) license in the state of Florida; current BLS/CPR certification.
- Experience in home health or community-based nursing strongly preferred; familiarity with Medicare regulations is beneficial.
- Strong communication, documentation, and time management skills, with the ability to work independently and collaboratively.
- Valid driver’s license, reliable transportation, and ability to travel to patient homes; ability to meet physical demands of in-home care.
