PACT Care Manager RN
DutiesProvides coordinated care across the life span to Veterans, families, caregivers, groups, populations, and communities.
Provides coordination and transition of care across the continuum of care in a variety of outpatient settings.
During each encounter, the PACT Care Manager RN focuses on patient safety and the quality of nursing care by applying appropriate nursing interventions.
Assists the patient to navigate the health care system, and evaluating patient outcomes.
Acts as a partner and advisor, assists and supports patients and families to optimally manage their health care.
They facilitate continuity of care using the nursing process, multidisciplinary collaboration, and coordination of appropriate health care services and community resources across the care continuum.
Systematic data gathering, assessment, appropriate nursing judgment, and evaluation of human responses to actual or potential health problems through such services as case finding, health teaching, health counseling, provision of care supportive to or restorative of life and well-being, and executing medical regimens.
The nurse's primary commitment is to the patient, whether an individual, family, group, or community.
The staff nurse interprets the philosophy and goals of the Nursing Service to others in the work setting.
Functions within established guidelines and demonstrates the ability to work effectively and develop therapeutic relationships with patients, their families and other professional and supportive personnel who provide patient care.
