The Home Health Field Registered Nurse (RN) plays a critical role in delivering high-quality, patient-centered care within the comfort of patients homes. This position involves comprehensive assessment, planning, implementation, and evaluation of nursing care tailored to individual patient needs, often managing complex medical conditions. The RN collaborates closely with interdisciplinary teams, patients, and their families to promote health, prevent complications, and support recovery or chronic disease management.
By providing education, emotional support, and clinical expertise, the RN empowers patients to achieve optimal health outcomes and maintain independence. This role ensures continuity of care outside traditional clinical settings, improving patient satisfaction and quality of life.
- Initial and ongoing comprehensive assessments and treatment of the patient s physical, functional, psychosocial, and environmental needs as evidenced by documentation, clinical records, case conferences, team reports, evaluations, and ADLs.
- Apply criteria for admission and re-certification to home health care to establish appropriate levels of care and the patient s eligibility.
- Develop, implement, and document the plan of care to ensure quality and continuity of care and revise the plan as necessary.
- Educate the patient and family regarding self-care techniques, nutrition, and dietary needs.
- Determine scope and frequency of services based on acuity and patient/family needs; assess caregiver ability to meet immediate needs upon admission and throughout care.
- Initiate appropriate preventative and rehabilitative nursing procedures.
- Apply concepts of infection control and standard precautions in coordinating and performing patient care activities to prevent contamination and transmission of disease.
- Provide clinical directions to the Home Health Aide and LPN/LVN to ensure quality and continuity of service.
- Prepare clinical and progress notes that demonstrate progress toward established goals.
- Inform physician and other personnel of changes in the patient s needs and outcomes of intervention, while evaluating patient and family response to care.
- Monitor assigned cases to ensure compliance with requirements of third-party payors.
- Provide effective communication to patients, family, team members, and other health care professionals.
- Current RN License
- Valid Driver’s License
- CPR Certification
- Valid Auto Insurance
- At least 2 years of Home Health RN experience preferred
- Home Care Home Base experience preferred
