Provides a seamless navigation process by serving as a liaison between patients and their caregivers and the clinical care team, to enhance the patient experience and satisfaction. Consistently coordinates and manages the patient's plan of care through the cancer continuum, while building relationships with patients and Providers. Works in collaboration with Nurse Navigator and other members of the interdisciplinary team to provide safe delivery of the patient's plan of care.
ResponsibilitiesWhat you will do
- Establishes and maintains close collaboration with clinical, business staff, and multidisciplinary team members to ensure seamless patient care coordination.
- Develops and maintains positive working relationships with key internal/external customers.
- Coordinates patient care through the cancer continuum by assisting with timely access to care, coordinating/scheduling appointments and services, and encouraging adherence to treatment plans, protocols, and follow up care.
- Builds awareness of the oncology navigator’s role by participating in marketing and community outreach efforts.
- Demonstrates knowledge, skill, and coordination to provide care and guidance to the cancer patient from screening to survivorship.
- Participates in the performance improvement process with the department leaders, making ongoing recommendations, providing updates on institution practices, involved in the patient/guest satisfaction.
- Maintains reasonably regular, punctual attendance consistent with Orlando Health policies, the ADA, FMLA and other federal, state and local standards.
