Supervising Nurse

The Supervising Nurse is a registered professional nurse who provides oversight of all patient/client care services and personnel. Ensures quality of patient/client care in the home. Coordinates care with the interdisciplinary team, patient/client, family, and referring agency. May also be called Clinical Manager, Director of Nurses, Director of Clinical Services, Director of Professional Services, or a similar title. Director of Nursing for a Home Healthcare company Lead the nursing team to improve and sustain the superior performance of the facility Plan and organize the daily functions of the Nursing department Use independent judgment and follow regulatory guidelines in the delivery of quality care Plan and organize day-to-day functions of resident care Anticipate, identify and analyze problems effectively and make appropriate recommendations or intervene as necessary Participate in the development and updating of the interdisciplinary plan of care Available 24/7 by phone, unless another qualified Registered Nurse (RN) has been scheduled to be available Complete pre-admission resident evaluations at their current place of care Travel throughout the area required

Job Qualifications

Education:

Graduate from an accredited diploma, associate, or baccalaureate nursing program. Computer Based Training for appropriate category(ies) of license. Licensure:
Active, unencumbered state license as a registered nurse, or a multi-state license issued by a Nurse Licensure Compact (NLC) member state. Current driver's license.

Experience:

Nursing skills as defined as generally accepted standards of practice. Excellent interpersonal skills and ability At least two (2) years of home care experience with one (1) year supervision experience to communicate effectively. Demonstrates proven decision-making skills. If required to make visits, proof of current CPR certification.

Reliable transportation and valid auto liability insurance.

Working Conditions:

Perform duties in an office environment. If required to make patient/client visits, may work in patients'/clients' homes in various conditions; possible exposure to blood, bodily fluids, and infectious diseases; ability to work a flexible schedule; ability to travel locally; some exposure to unpleasant weather; PRN emergency calls. Supervising Nurse or Alternate must be available at all times either in person or via telecommunications.

Physical and Mental Effort:
Prolonged sitting required, with the ability to operate office equipment which may include heavy lifting, bending, and standing. Requires the ability to handle stressful situations in a calm and courteous manner at all times. If required to make patient/client visits, prolonged standing and walking required, with ability to lift up to fifty pounds and move patients/clients. Requires working under some stressful conditions to meet deadlines and patient/client needs, and to make quick decisions and resource acquisition

Essential Functions:

Coordinates patient/client care. Coordinates and integrates care provided by Agency staff, including contractual providers. Ensures communication with all physicians involved in a patient's/client's plan of care and other healthcare practitioners. Integrates orders from all relevant physicians involved into the plan of care and ensures the orders are approved by the responsible physician. Coordinates referrals. Participates in activities relevant to professional services including development of qualifications.

Recommends resources needed to provide care. Makes patient/client and personnel assignments. Assures that patient/client needs are continually assessed. Assures reassessment of the patient's/client's needs is performed by the appropriate healthcare professional when there is a significant health status change in the patient's/client's condition, at the physician's request, and after hospital discharge. Assures the development, implementation, and updates of the individualized plan of care in partnership with the patient/client, representative (if any), and caregiver(s).

Ensures provision of services as ordered by the physician as indicated in the plan of care, including patient/client and caregiver education and patient/client, caregiver, and family counseling. Prepares clinical notes as appropriate. Assumes responsibility for continuity, quality, and safety of services delivered in compliance with state and federal regulations Collaborates on development and implementation of policies and procedures which guide and support the provision of care.

Collaborates with Agency leadership and appropriate staff to develop the organizational plan as it pertains to patient/client care.
