• The
  Clinical Manager will coordinate and oversee all direct care patient services provided by clinical personnel.
• You will conduct assessment and reassessment of patients, update plan of care, evaluate patients' needs and adhere to LeeKO's policies and procedures.
• Monitor and coordinate caregivers' assignments.
• Review referrals for acceptance ensuring patients meet admission criteria.
• Admit patients and assign appropriate clinicians based on the patient's needs.
• Effectively encourages high performance of service delivery and by being available to clinicians during business hours.
• Ensure documentation is in compliance with regulatory agencies and requirements of third‑party payers.
• Ensure final audits/billing are completed timely and in compliance with Medicaid/Medicare and third‑party payers regulations.
• Coordinate communication between team members/attending physicians/caregivers to ensure the appropriateness of care and outcome planning.
• Conduct interview and assist with on‑boarding of newly hired clinicians.
• Conduct annual performance evaluation.
• Conduct QAPI and annual CEMP review.
• Perform other related duties as requested by upper management.

• Graduate of an accredited School of Nursing.
• Current state or multi‑state license as a Registered Nurse.
• Current CPR.
• Valid driver’s license, with reliable transportation.
• Two years as a Registered Nurse in a home care.
• Preferred at least one year of home health management experience.
