We are seeking an experienced and detail-oriented Home Health Nurse to join our team and play a key role in delivering exceptional patient care.

Looking for visit nurses! Wound Care experience a plus!

• Develop individualized patient care plans based on assessment findings, physician orders, and patient goals.
• Educate patients and families on disease processes, medication management, safety, and self-care techniques.
• Coordinate multidisciplinary services, including therapy, nursing, and aide support.
• Ensure compliance with documentation standards, agency policies, and regulatory requirements.
• Communicate effectively with physicians, case managers, and other care team members.
• Provide compassionate, patient-centered care in the home setting.

• Current nursing license in the state of FL (required).
• Minimum 1-2 years of home health nursing experience with strong knowledge of OASIS documentation.
• Strong organizational, time management, and critical thinking skills.
• Excellent communication and interpersonal skills.
• Proficiency in EMR/home health documentation systems (we use Kinnser/Well Sky, but Axxess, or similar experience preferred).
• Reliable transportation and valid driver's license.
