Center Well Senior Primary Care is seeking a Social Worker to support the HRPM program for the top 5% of patients with complex psychosocial needs. You will perform psychosocial assessments, care coordination, and resource navigation, collaborating with Care Coaches and PCPs to improve engagement and outcomes.
The role is hybrid, requiring onsite clinic presence 2–3 days per week with some days remote. A MSW is required, and bilingual English/Spanish skills are essential for diverse patient care.
