Driver - Participant Care

Innov Age Driver – Participant Care provides transportation for frail and elderly participants from their home to Day Centers and appointments during the day, as well as field trips. The driver also delivers in‑home meals and provides delivery and pick‑up of supplies and medications for participants, in addition to performing pre/post trip inspections.

Physical attendance at an Innov Age facility and/or an Innov Age participant home is essential to performing the job duties. The work schedule is Monday through Friday, with some Saturday and holiday operations that require working on a rotating schedule basis.

• Drive Innov Age vehicles safely and responsibly, utilizing defensive driving skills and following Innov Age policies, procedures, and assigned routes.
• Load and unload participants in and out of Innov Age vehicles and assist participants into and out of their homes in adherence with Innov Age policies and procedures.
• Transport participants to all pre‑arranged appointments.
• Deliver in‑home meals, supplies, and medications, ensuring correct participants receive the correct parcels.
• Assist participants into their homes and help with simple tasks when necessary and appropriate (e.g., helping with outerwear and moving items that obstruct walking paths).
• Keep Dispatch updated on all events of assigned routes.
• Maintain safety and cleanliness of assigned vehicle and coordinate scheduling of van maintenance with the Manager of Transportation.
• Assist other drivers with the loading and unloading of participants arriving and exiting the center.
• Participate in interdisciplinary team settings in the formulation of Plans of Care for Innov Age PACE program participants.
• Perform additional work functions and duties as assigned.

• Previous DOT and defensive driving experience.
• One year of experience working with the frail or elderly.
• High school diploma or GED, or one year of experience within the realm of the position’s duties and responsibilities substituting for a high school diploma.
• Ability to lift, push, pull, and carry up to 75 pounds.

• California (San Bernardino and Sacramento Centers):
  Valid Class B Commercial Driver’s License with Passenger endorsement (Class A acceptable), good driving record, at least 21 years old.
• California (Los Angeles), Colorado, Florida, New Mexico, Pennsylvania, and Virginia:
  Valid driver’s license, good driving record, at least 21 years old.
• Drivers must maintain a valid state‑issued medical examiner’s certificate when required by applicable state law.

• Knowledge of how to use and maintain positioning equipment.
• Experience transporting the elderly.
• Six months experience transporting individuals with special mobility needs.

Pay range: $19.85 - $19.85 per hour. Pay may vary depending on job‑related factors such as work location, experience, education, certifications, and internal equity.
